Empty Cupboard Dark Mild

I had a Nottingham yeast cake from a previous beer and decided to brew somethign simple, dark, and mild. I used leftover ingredients.

Type: Extract w/grain Size: 5.5 gallons
Color:
91 HCU (~33 SRM)
Bitterness: 13 IBU
OG: 1.045 FG: 1.008
Alcohol: 4.7% v/v (3.7% w/w)
Water: 1/2 crushed campdem tablets in the water to remove chlorine and chloramine.
Grain: 1 lb. American chocolate
Boil: 60 minutes SG 1.038 6.5 gallons
3.3 lb. Dark malt extract
2.5 lb. Light dry malt extract
Hops: .5 oz. Kent Goldings (5% AA, 60 min.)
.5 oz. Kent Goldings (5% AA, 15 min.)
Yeast: Danstar Nottingham yeast cake.
